To Create and Submit an Appendix E for a Multi-State Project:<\/h2>\n

  1. Go to the National Information Management Support System at http:\/\/www.nimss.org\/<\/a><\/li>\n
  2. Enter your login and password.
    \n[NOTE: If you don’t have a login ID, click on “New User?” to register. If you forgot, please use the “Forgot Password” link. Please do not re-register.<\/b>]<\/li>\n
  3. From the left menu, select Participants then select Draft\/Edit Particip. Info<\/li>\n
  4. Select “Draft New”<\/li>\n
  5. Select the Project Number<\/li>\n
  6. Select User<\/li>\n
  7. Fill out the form.\u00a0 \u00a0[NOTE:\u00a0 Do not submit form with all zeroes in the FTE section.\u00a0 It will be returned to you.]<\/li>\n
